Special Versions - Shared Programs (registry customizations shared by other users)

To receive notification of CDEMS updates, click add me to the CDEMS list (e-mail: jackie@cdems.com). Simply use "CDEMS e-mail List" as the subject line and include your name, e-mail address and clinic name/location in the message. You'll be the first to know what's happening with these programs!



CDEMS PROGRAMS

Save the CDEMS program files corresponding to your version of Microsoft Access* to a CDEMS folder on your computer. Double-click on each file in the list below (note: import tool is optional and used only if converting from DEMS to CDEMS). Save each file to the CDEMS folder. After the compressed files are downloaded, double-click on each file > Extract > Self-Extract > Execute to uncompress. Double-click the uncompressed data entry program file (cdem_3-4-08) to begin using CDEMS. The Data Entry User Guide provides more detailed instructions for downloading files, initial setup, and customizing CDEMS. While you're downloading programs, scroll to the Documentation section below to pick up this handy guide.

CAUTION! Existing users should NOT download a new cdem_dta file; installing a new data file will overwrite your current data! Re-install or upgrade, only the Data Entry and/or Reports programs.

Access 2007 Files

* Program files must correspond to your version of Microsoft Access -- check your version by clicking Start > Programs from your computer desktop. Find Microsoft Access in the program list and note the color of the Access icon "Key". A maroon key = Access 2000/02/03; a yellow key = Access 97. Access-2007 also has a maroon icon key. Access-2007 requires the 2007 version of the Data Entry program but is compatible with 2k versions of the data file and Reports program.

TIPS FOR RESOLVING NEW VERSION ISSUES...

  • If your system prevents CDEMS program files from opening the first time, right-click on the file name in your CDEMS folder > Properties > click "Unblock" in the Security area to jumpstart the program.

  • If your firewall blocks .EXE files, please e-mail Jackie (jackie@cdems.com) to receive the uncompressed files on a CD.

  • If your data file is located somewhere different than the CDEMS program files, the new program on first opening must be pointed to the location of the cdem_dta.mdb file on another computer or server. Or, from the Data Entry or Reports opening window, click on Tools > Database Utilities > Linked Table Manager. Place a checkmark in "Always prompt for new location" box > click "Select All" > Press OK and navigate to location of cdem_dta file.

  • If encountering unexpected errors on opening the new Data Entry program, check out possible reference issues due to differences in versions of Microsoft Access:
    1. From the Data Entry program, verify references by pressing the F11 function key > click on Modules > New
    2. Click Tools in the menu bar > References. Look for a "missing" reference in the list (likely a Microsoft Graph Object Library reference) > uncheck the missing reference
    3. Scroll down the list and select the matching Graph Object Library version available on your computer (likely Microsoft Graph 9.0, 10.0, or 11.0 Object Library).
    4. Click on "X" in upper right corner to close the module > Save with the default module name when prompted.
    5. Close program and re-open

CDEMS FOR 2008 WA STATE COLLABORATIVE

To begin a new registry for the Collaborative, begin by downloading the CDEMS Data Entry program and a data file containing measures customized for each particular Collaborative track [NOTE: the Pediatric Obesity track has a special version of both the Data Entry program and data file -- scroll to end of file list].

STEP #1 - Decide where to install CDEMS (single computer or network server). Create a CDEMS folder on that computer.
STEP #2 - Select the CDEMS Data Entry and Reports files from the list below that match your version of Microsoft Access (2k or Access-07). Double-click on the file; save to the CDEMS folder on your computer.
STEP #3 - Locate the data file (cdem_dta) that corresponds to your condition track for the Collaborative. There's only one version of the data file, so simply double-click on the appropriate file and save to your CDEMS folder.

CAUTION! Existing users should NOT download a new cdem_dta file; installing a new data file will overwrite your current data!

The good news is that Collaborative tracking measures can easily be copied into an existing registry. Download the Excel file for the appropriate track and follow instructions in the worksheet for copying the measurement set into the appropriate CDEMS tables.

STEP #4 - After downloading, locate the zipped files in your CDEMS folder; double-click on each file > Extract > Self-Extract > Execute to unzip. Double-click the unzipped Data Entry file to begin using CDEMS. The Data Entry User Guide provides detailed instructions for downloading files, initial setup, and customizing CDEMS. While you're downloading programs, scroll to the Documentation section below and pick up this handy guide.

CDEMS+SQL


CDEMS+SQL looks and acts just like native CDEMS. The main difference is that the data stored in the MSAccess cdem_dta.mdb file is now stored in a SQL Server database. Technical support for CDEMS+SQL is not offered. While the basic CDEMS SQL program files are provided, your organization must have the technical capability to manage the SQL database. Below are the files needed to install the SQL version of CDEMS; refer to the CDEMS+SQL Guide for complete instructions:

CDEMS DOCUMENTATION

The CDEMS User Guide is available in four sections to minimize file size and download time. If you're new to CDEMS, the first section provides a great overview of the program and step-by-step instructions for getting started. Here's how to download the Guide:

  1. Click the desired Guide section(s) to download into your CDEMS folder
  2. SAVE the file to your CDEMS folder
  3. When download is complete, double-click on the saved file (a Quarterdeck Zip-It window appears > click Extract Files > Self Extract > Execute > Close)
  4. Double-click the extracted file to open the document.

ELECTRONIC LAB INTERFACE PROGRAMS

Lab interface downloads require setting up an account with your lab company, complete with password and installation of appropriate software for receiving lab results electronically. Lab codes must also be entered into CDEMS tlkpCrosswalk before the lab interface is functional. Please refer to the CDEMS User Guide [Electronic Lab Interface section] for detailed instructions, or e-mail Jackie (jackie@cdems.com) for additional personalized assistance.

  1. HL7 Generic

    * HL7 - Cdems 2k
    * HL7 - Cdems 2007

    HL7 is a generic interface that processes any HL7-formatted lab file. The 2k version works with Access-2003 and earlier; the 2007 version with Access-2007. Place the HL7 interface WITHIN the lab results folder and link the CDEMS data file.

    The calculation of eGFR (Glomerular Filtration Rate) is embedded in this interface, based on serum creatinine results reported in the lab file. Entries for "eGFR" lab in CDEMS tlkpCrosswalk and tlkpSetup must be present for this calculation. Additionally, patient's date of birth, gender and ethnicity must be recorded in CDEMS or eGFR is recorded as "UTD" (unable to determine). Since ethnicity is a key factor in this calculation, African American ethnicity must be recorded as "Black" or the appropriate interface code modification made to handle other ethnicity names.

SPECIAL VERSIONS

Customized versions of CDEMS programs may be downloaded here. These specialized adaptations shared by other CDEMS users are provided without documentation. It's possible to review the measurement set or modify a shared form, for example, to use as a building block in the development of a new chronic condition track. Basically these programs are available "AS IS" and must be explored on your own though very general guidance may be available by e-mail (jackie@cdems.com).

Download Instructions: Double-click on each file you wish to download > save to your CDEMS folder. After the compressed files are downloaded, double-click on each file > click Extract > Self-Extract > Execute to uncompress. Double-click the uncompressed data entry program file to begin using.

CAUTION! Do NOT download a data file into the same CDEMS folder if you have an existing CDEMS registry. Doing so will overwrite the existing cdem_dta.mdb file and wipe out your data.

PECS to CDEMS Conversion tool

The PECS Conversion tool, developed by the West Virginia University Office of Health Services Research, provides a step-by-step mechanism for remapping exported PECS data into CDEMS format. CDEMS - CA Safety Net Institute "SEED" Project

  • CDEMS-2k Data Entry program
    This is the standard CDEMS data entry program placed here for your convenience in downloading all CDEMS files from one location. There are no customizations in this program for the SEED project.
  • CDEMS-2k SEED Reports program
    The SEED reports (v.2-15-08) contains an updated monthly SEED summary report template for Diabetes and special queries needed to report on SEED measures. Cardiovascular prevention queries and a new query to count pneumonia vaccine regardless of age and time period are updated in the 2-15-08 version . A list query showing service referrals made/completed was added to the program. In addition, the Create-a-Report template features an option to generate user-defined summaries by insurance payer.
  • CDEMS SEED Data file
    This data file contains the customized measurement setup for the SEED project. CAUTION: DO NOT DOWNLOAD THIS FILE IF YOUR REGISTRY IS ALREADY STARTED. Installing this file will overwrite any already existing cdem_dta.mdb file and wipe out your data!

CDEMS - HIV

  • CDEMS-2k HIV Data Entry program
    This file contains the CDEMS data entry program with customized HIV progress note forms and graphs.
  • CDEMS-2k HIV Reports program
    This folder contains an adaptation of the 7-14-04 CDEMS reports program with queries for correlating adherence education with antiviral adherence rates.
  • CDEMS-2k HIV Data file
    The HIV data file contains the customized HIV setup with 3 sample patients.
