Special Versions - Shared Programs (registry customizations shared by other users)

CDEMS PROGRAMS

Save the CDEMS program files corresponding to your version of Microsoft Access* to a CDEMS folder on your computer. Double-click on each file in the list below. Save each file to the CDEMS folder on your computer or server. After the compressed files are downloaded, double-click on each saved file > Extract > Self-Extract > Execute to uncompress. Double-click the uncompressed data entry program file (cdem_DataEntry_6-7-10) to begin using CDEMS. The Data Entry User Guide provides more detailed instructions for downloading files, initial setup, and customizing CDEMS. While you're downloading programs, scroll to the Documentation section below to pick up this handy guide.

CAUTION! Existing users should NOT download a new cdem_dta file; installing a new data file will overwrite your current data! Re-install or upgrade only the Data Entry and Reports program files.

Access 2000/02/03 File Versions

  • Data Entry: cdem2k_DataEntry_6-7-10**

  • Optional aggregate reporting module: PowerTools2k_4-23-05 * Program files must correspond to your version of Microsoft Access -- check your version by clicking Start > Programs from your computer desktop. Find and open Microsoft Access in the program list. Click Help > About Microsoft Access to view your program version (Access 2007/2010 may actually display the version in the program title). Access 2000, 2002 and 2003 all use CDEMS 2k file formats.

    ** Data Entry v.6-7-10 contains a customized Asthma progress note (2A) summarizing # of visits in the last 12 mos, steroid bursts in the last 12 mos, and max peak flow recorded ever. The form also displays a short list of health education topics related to asthma. This program version is a minor update to the 10-12-09 Data Entry program and includes only a progress note customization of interest to those tracking asthma patients. If not tracking asthma, there is no reason to upgrade from the 10-12-09 Data Entry program.

    *** Reports v.7-30-10 contains a minor patch to repair the formatting of averages in summary reports created through the Create-a-Report template in Access 2007 & 2010 versions of the program only.

    **** The cdem_dta file was updated on 6-9-2010 to include the measurement setup for Asthma used in the 2008 WA State Collaborative and matching NHLBI 2007 asthma guidelines. New registry installations will automatically have access to the asthma setup, as well as adult preventive health, diabetes, and cardiovascular. Once installed, any of the pre-defined measures can be adapted to fit your program. Registry users wishing to add asthma measures to an existing data file should copy the setup; NEVER DOWNLOAD A NEW DATA FILE. Click Asthma SetUp for the Excel file with asthma measures and instructions for appending to an existing registry.

    Access 2007/2010 File Versions

    Note that only the Data Entry program is an Access 2007/2010 .accdb file. The 2k versions of the Reports program and data file are compatible with all Access versions.

    Different combinations of operating systems, security settings, and Access versions may create a variety of installation issues with the CDEMS files. Several file formats are provided here to work around the many possible configuation permutations. Try downloading option #1 (zipped files) first for each CDEMS program file. If blocked or a 64-bit error prevents unzipping the files, try downloading option #2 (executable files) for each CDEMS program file, and finally try option #3 (unzipped files). If all these options fail, a CD with unzipped files can be requested -- e-mail (jackie@cdems.com).

    Refer to Access 2007-2010 Tips for assistance. These instructions for Access 2007/2010 installations are also included in the Data Entry Guide.

    CDEMS FOR 2009 WA PATIENT-CENTERED MEDICAL HOME COLLABORATIVE

    To begin a NEW registry for the Collaborative, begin by downloading the CDEMS Data Entry program and the Medical Home data file below.

    STEP #1 - Decide where to install CDEMS (single computer or network server). Create a CDEMS folder on that computer.
    STEP #2 - Scroll up to the "Programs" section and select the CDEMS Data Entry file that matches your version of Microsoft Access (2k or Access-07). Double-click on the file; save to the CDEMS folder on your computer.
    STEP #3 - If you are installing CDEMS for the first time, double-click on the Medical Home data file below; save to the CDEMS folder on your computer.

    CAUTION! Existing users should NOT download a new cdem_dta file; installing a new data file will overwrite your current data! 2009 Medical Home measures can easily be added through the Setup Wizard or copied and appended to an existing registry from the Crosswalk and Setup tables in the Excel Setup file below.

    STEP #4 - After downloading, locate the zipped files in your CDEMS folder; double-click on each file > Extract > Self-Extract > Execute to unzip. Double-click the unzipped Data Entry file to begin using CDEMS. You may need to point or relink the program to the location of the data file. The Data Entry User Guide provides detailed instructions for downloading files, linking to the data file, initial setup, and customizing CDEMS. While you're downloading programs, scroll to the Documentation section below and pick up this valuable guide.

    STEP #5 - Download the CDEMS PCMH Reports program (v. 11-16-09). The program contains special queries and templates for producing PCMH adult prevention and pediatric wellness reports as well as the monthly IPIP diabetes report. Double-click on the reports file below and save to the CDEMS folder on your computer. Double-click on the zipped reports file in your CDEMS folder > Extract > Self-Extract > Execute to unzip. Double-click the unzipped rpts2k_PCMH_111609 file to begin using. You may need to point or relink the program to the location of the data file. The CDEMS PCMH Reports Guide provides detailed instructions for producing PCMH and IPIP reports.

    CDEMS+SQL


    CDEMS+SQL looks and acts just like native CDEMS. The main difference is that the data stored in the MSAccess cdem_dta.mdb file is now stored in a SQL Server database. Below are the files needed to install the SQL version of CDEMS; refer to the CDEMS+SQL Guide for complete instructions.

    * Important Considerations:

    1. The SQL Data Entry program works with Microsoft Access 2003 and earlier. Code changes required to produce CDEMS graphs when using MS-Access 2007/2010 have not been made in the CDEMS+SQL Data Entry program.

    2. The CDEMS+SQL programs have been available since 2005. While the SQL versions of the CDEMS Data Entry and Reports programs are fully functional, neither program has up-to-date features and improvements made since 2005.

    3. It's expected that your organization has internal capability to manage technical issues and maintenance of the SQL database. Technical support for CDEMS+SQL is strictly fee-for-service, 1 hr minimum.

    CDEMS DOCUMENTATION

    The CDEMS User Guide is available in sections to minimize file size and download time: Data Entry, Reports, Lab Interface, Appendix and the PowerTools Guide if opting to use the optional aggregate reporting module. If you're new to CDEMS, the Data Entry Guide provides a great overview of the program and detailed installation and initial setup instructions to get a registry started today.

      Contains a program overview, technical requirements,installation/startup instructions, customizations & data entry/edit/print functions. The 0211 version of the Guide includes detailed installation instructions and troubleshooting tips for Access 2007/2010.
    • CDEMS Reports Guide (v.0211) (23 p.)
      Detailed instructions for running and creating reports to analyze CDEMS data. Includes instructions for developing user-defined summaries with the Create-a-Report template and user-defined intervention lists with Create-a-List template. Also contains CDEMS table descriptions.
    • CDEMS Electronic Lab Interface Guide (13 p.)
      Lab interface overview, setup instructions, testing, maintenance, troubleshooting tips...
    • PowerTools Guide (5 p.)
      A short instructional guide for using PowerTool programs: GROUPER for aggregating data files to run reports across multiple sites and LOGGER for monitoring who's currently accessing the CDEMS data file.

    ELECTRONIC LAB INTERFACE PROGRAMS

    Lab interface downloads require setting up an account with your lab company, complete with password and installation of appropriate software for receiving lab results electronically. Lab codes must also be entered into CDEMS tlkpCrosswalk before the lab interface is functional. Please refer to the CDEMS User Guide [Electronic Lab Interface section] for detailed instructions, or e-mail Jackie (jackie@cdems.com) for additional personalized assistance.

    • HL7 Generic

      * HL7 - CDEMS-2k
      * HL7 - CDEMS-2007-2010

      HL7 is a generic interface that processes any HL7-formatted lab file. The 2k version works with Access-2003 and earlier; the 2007 version with Access 2007/2010. Place the HL7 interface WITHIN the lab results folder and relink to your CDEMS data file.

      The calculation of eGFR (Glomerular Filtration Rate) is embedded in this interface, based on serum creatinine results reported in the lab file. Entries for "eGFR" lab in CDEMS tlkpCrosswalk and tlkpSetup must be present for this calculation. Additionally, patient's date of birth, gender and ethnicity must be recorded in CDEMS or eGFR is recorded as "UTD" (unable to determine). Since ethnicity is a key factor in this calculation, African American ethnicity must be recorded as "Black" or the appropriate interface code modification made to handle other ethnicity names.

    SPECIAL VERSIONS

    Customized versions of CDEMS programs may be downloaded here. These specialized adaptations shared by other CDEMS users are provided without documentation. It's possible to review the measurement set or modify a shared form, for example, to use as a building block in the development of a new chronic condition track. Basically these programs are available "AS IS" and must be explored on your own though very general guidance may be available by e-mail (jackie@cdems.com).

    Download Instructions: Double-click on each file you wish to download > save to your CDEMS folder. After the compressed files are downloaded, double-click on each file > click Extract > Self-Extract > Execute to uncompress. Double-click the uncompressed data entry program file to begin using.

    CAUTION! Do NOT download a data file into the same CDEMS folder if you have an existing CDEMS registry. Doing so will overwrite the existing cdem_dta.mdb file and wipe out your data.

      CDEMS - KS QOC Project

      Custom CDEMS data file and Reports program with special queries & summary reports for diabetes, hypertension, hyperlipidemia and prediabetes Quality of Care projects in Kansas. Upgrade released 5-5-11.

      DEMS to CDEMS Conversion tool

      Use the DEMS to CDEMS conversion program to upgrade your DEMS data file to the CDEMS format. Create a CDEMS folder on your computer and download a new CDEMS data file from this web site into the CDEMS folder. Place a copy of the DEMS data file into the same folder. Run the conversion tool to export DEMS data into the new CDEMS format. The DEMS data is not changed during the conversion process.       PECS-to-CDEMS Conversion tool

      The PECS Conversion tool, developed by the West Virginia University Office of Health Services Research, provides a step-by-step mechanism for remapping exported PECS data into CDEMS format. The 2007 conversion tools works with either a regular or anonymous export from PECS. Click PECS-to-CDEMS Conversion to download the conversion tool, a blank data file, and detailed instructions. Save the PECS Conversion file to your desktop and work through the instructions with the files within this folder. When the conversion process is complete, move a copy of the cdem_dta.mdb file to your CDEMS folder to link with the CDEMS Data Entry and Reports programs.


      CDEMS - CA Safety Net Institute "SEED" Project


      • This is the standard CDEMS data entry program placed here for your convenience in downloading all CDEMS files from one location. There are no customizations in this program for the SEED project.
      • CDEMS-2k SEED Reports program
        The SEED reports (v.2-15-08) contains an updated monthly SEED summary report template for Diabetes and special queries needed to report on SEED measures. Cardiovascular prevention queries and a new query to count pneumonia vaccine regardless of age and time period are updated in the 2-15-08 version . A list query showing service referrals made/completed was added to the program. In addition, the Create-a-Report template features an option to generate user-defined summaries by insurance payer.
      • CDEMS SEED Data file
        This data file contains the customized measurement setup for the SEED project. CAUTION: DO NOT DOWNLOAD THIS FILE IF YOUR REGISTRY IS ALREADY STARTED. Installing this file will overwrite any already existing cdem_dta.mdb file and wipe out your data!

      CDEMS - HIV

      • CDEMS-2k HIV Data Entry program
        This file contains the CDEMS data entry program with customized HIV progress note forms and graphs.
      • CDEMS-2k HIV Reports program
        This folder contains an adaptation of the 7-14-04 CDEMS reports program with queries for correlating adherence education with antiviral adherence rates.
      • CDEMS-2k HIV Data file
        The HIV data file contains the customized HIV setup with 3 sample patients.

      CDEMS FOR 2008 WA STATE COLLABORATIVE

      Below are data files and Excel Setup files for the Asthma, Diabetes, Hypertension and Medical Home tracks for the 2008 WA State Collaborative.
